Need help with story! descrição below :)

A girl is dating a guy that she loves but another girl wants him and threatens to reveal a secret to the guy unless she breaks up with him.

Now I need a secret but something different, nothing cheating related just something she would be too scared to tell him but something he would easily forgive. Please help!

Maybe something to do with her past? Maybe there is something she regrets like maybe she let someone she loves die or maybe she went through a traumatic event like something that would put her in a hospital like suicide or something like because she has a mental disability?
