If you get a medal when you're online, do you get a notification?

You don’t. One dia you just come to your perfil and see that the number seguinte to medals has changed... And you are like “WOW!” :D

I did receive a medal some time atrás but, to be honest, I don't remember getting any notification about it.
I found about it the seguinte dia and at first didn't know what did I get it for. XD
But it could change now so I'm not sure to tell you the truth.^^'
